The hip-shelf procedure for hip dysplasia in older children shows poor long-term outcomes, including pain and degenerative changes. Alternative surgical stabilization methods are recommended for dysplastic acetabula.

    Area of Science:

    • Orthopedic Surgery
    • Pediatric Orthopedics

    Background:

    • The hip-shelf procedure is a surgical technique used to address acetabular dysplasia in older children.
    • Interest has resurged in evaluating the long-term efficacy of this procedure.

    Observation:

    • A study evaluated long-term results (minimum 10 years) of the hip-shelf procedure in 29 hips (27 patients).
    • Average follow-up was over 22 years.
    • Pain, limping, and degenerative changes were common at follow-up.

    Findings:

    • Complications occurred in 21 hips, with 14 requiring further surgery.
    • Of 21 hips assessed, only 1 was rated excellent, 8 satisfactory, and 12 poor.
    • Long-term outcomes suggest significant limitations and complications.

    Implications:

    • The hip-shelf procedure may not be as safe or reliable as previously thought for treating acetabular dysplasia.
    • Consideration of alternative surgical stabilization procedures for dysplastic acetabula is advised.
    • This study highlights the need for cautious evaluation of long-term results for orthopedic procedures.
