    Background:

    • Longstanding pulmonary congestion from heart valve disease can affect airway mechanics.
    • Airway rigidity may impair lung distensibility and compressibility.

    Purpose of the Study:

    • To assess changes in lung volumes and pressure-volume (P-V) relationships after aortic or mitral valve replacement.
    • To investigate the impact of valve surgery on airway compliance and conductance.

    Main Methods:

    • Measurements of lung volumes, static P-V relationships, and conductance-pressure relationships were taken before and 1-2 years after surgery.
    • Included 14 patients undergoing single aortic or mitral valve replacement.

    Main Results:

    • Significant increases in total lung capacity (TLC), functional residual capacity (FRC), residual volume (RV), and static tidal compliance were observed post-operation.
    • P-V curves showed altered shape and position, with leftward shifts in aortic disease patients and crossing patterns in mitral disease patients.
    • Airway conductance increased at high lung volumes, suggesting reduced airway rigidity after relieving pulmonary congestion.

    Conclusions:

    • Aortic or mitral valve replacement can significantly improve lung mechanics.
    • Reduced pulmonary congestion post-surgery leads to less rigid airways, enhancing lung distensibility and compressibility.
    • These findings highlight the beneficial effects of valve surgery on respiratory function.
