Pancreatic necrosis in progressive systemic sclerosis

A A Abraham, A Joos

    Annals of the Rheumatic Diseases

    Summary

Progressive systemic sclerosis can cause fatal pancreatic necrosis due to acute arteritis. This case highlights active vascular lesions in the kidneys and pancreas, leading to severe outcomes.

    Area of Science:

    • Pathology
    • Nephrology
    • Gastroenterology

    Background:

    • Progressive systemic sclerosis (PSS), a multisystem autoimmune disease, is characterized by fibrosis and vascular abnormalities.
    • Renal and pancreatic complications are known but often underdiagnosed manifestations of PSS.
    • Arteritis, inflammation of arteries, can affect various organs in PSS, leading to ischemia and necrosis.

    Observation:

    • A patient with PSS presented with severe hypertension and renal involvement, confirmed by biopsy showing active vascular lesions.
    • Autopsy revealed inactive renal lesions with characteristic concentric fibrosis.
    • The pancreas exhibited significant vascular pathology, with both chronic proliferative and acute arteritic changes.

    Findings:

    • Fatal pancreatic necrosis was directly attributed to extensive acute arteritic changes in the pancreatic vasculature.
    • The disparity between active vascular lesions in the initial presentation and the inactive renal lesions at autopsy suggests disease progression and organ-specific vascular involvement.
    • The pancreatic vascular lesions demonstrated a mixed pattern of chronic and acute inflammation.

    Implications:

    • This case underscores the critical role of vascular involvement in the pathogenesis of PSS complications.
    • Early recognition and management of arteritis in PSS may be crucial for preventing severe organ damage, including pancreatic necrosis.
    • Further research into the specific mechanisms of pancreatic vascular damage in PSS is warranted.
