    Background:

    • Gout is a metabolic disorder characterized by hyperuricemia.
    • Kidney dysfunction is a known complication of gout, but its specific mechanisms require further elucidation.

    Purpose of the Study:

    • To investigate and compare renal function in patients with gout versus normouricaemic controls.
    • To identify specific markers of renal impairment in gout patients.

    Main Methods:

    • Compared renal function (glomerular filtration rate, urine concentrating ability, urine pH, titratable acid, ammonium excretion, urate clearance) in 51 gout patients and 51 controls.
    • Assessed renal function before and after an acid load challenge.

    Main Results:

    • Gout patients exhibited significantly reduced glomerular filtration rate and urine concentrating ability compared to controls.
    • Gout patients excreted urine with a lower pH, higher titratable acid, and reduced ammonium excretion, particularly after an acid load.

    Conclusions:
