    Summary

    Overwhelming postsplenectomy infection risk is high in infants. This study found decreased pneumococcal opsonic activity in children splenectomized shortly after birth, supporting vaccination and prompt antibiotic use.

    Background:

    • Overwhelming postsplenectomy infection (OPSI) poses a significant risk, particularly in young children.
    • The risk is considered highest for children undergoing splenectomy before one year of age.
    • This study investigates immune function in infants splenectomized neonatally.

    Observation:

    • Three children who underwent splenectomy within two days of birth were assessed.
    • Immune function, specifically serum opsonic activity against Streptococcus pneumoniae, was evaluated.
    • Other immunological parameters were also examined.

    Findings:

    • All three patients exhibited decreased opsonic activity towards pneumococcus.
    • No other significant immunological abnormalities were detected.
    • These findings highlight a specific immune deficit post-neonatal splenectomy.

    Implications:

    • Splenectomized patients, especially those with early-life surgery, require prophylactic measures.
    • Pneumococcal polysaccharide vaccination is recommended for all splenectomized individuals.
    • Early and adequate antibiotic therapy is crucial for managing any infection in these vulnerable patients.
