    • Twelve cases of cranial settling in rheumatoid arthritis patients were reviewed.
    • Patients presented with varying degrees of CVJ instability.
    • Two patients experienced severe neurological deficits necessitating surgical intervention.

    Findings:

    • Surgical treatment was performed in two cases with significant neurological compromise.
    • The underlying cause is the collapse of the CVJ supporting structures.
    • Early recognition and management are crucial for preventing severe outcomes.

    Implications:

    • Surgical management can be effective in mitigating severe neurological sequelae.
    • Understanding the biomechanics of CVJ collapse in RA is vital.
    • Further research into non-surgical management and preventative strategies is warranted.
