    Summary

    An intermittent thiazide regimen using bendrofluazide effectively prevented new kidney stones in recent stone formers. This treatment also reduced stone recurrence rates in patients with existing stones.

    Background:

    • Kidney stones affect a significant portion of the population.
    • Recurrent stone formation poses a substantial health burden.
    • Thiazide diuretics are commonly used for stone prevention.

    Purpose of the Study:

    • To evaluate the efficacy of an intermittent thiazide regimen in preventing new stone formation and reducing recurrence in recent stone formers.

    Main Methods:

    • A cohort of 17 recent kidney stone formers received bendrofluazide (5 mg daily) with potassium supplements.
    • Treatment duration ranged from 15 months to 8 years (mean 4 years, 2 months).
    • Patients were monitored for new stone formation and recurrence rates.

    Main Results:

    • No new stones formed in 8 patients without initial radiologic evidence of stones (previous rate: 0.59 incidents/patient-year).
    • Stone recurrence rate decreased from 0.45 to 0.34 incidents/patient-year in 9 patients with initial stones.
    • Mild side effects were noted in 2 patients, resolving upon treatment cessation.

    Conclusions:

    • Intermittent thiazide therapy with bendrofluazide is effective in preventing kidney stone formation and reducing recurrence.
    • The regimen is generally well-tolerated with minimal side effects.
