    Summary

    Quinidine can trigger digitalis toxicity in patients on long-term digoxin therapy. This rare condition, marked by bidirectional tachycardia, resolves upon discontinuing digoxin.

    Background:

    • The supraventricular bidirectional tachycardia was characteristic of digitalis toxicity.
    • Discontinuation of digoxin led to the resolution of both clinical and ECG abnormalities.
    • The event was attributed to digitalis toxicity, not quinidine intolerance.
