    Summary

    Vaginal agenesis with a functional uterus presents challenges. Surgical creation of a neovagina can lead to severe morbidity, suggesting alternative approaches may be necessary for patient well-being.

    Background:

    • Vaginal agenesis with a functional uterus is a rare condition.
    • Current literature often recommends surgical neovagina creation to preserve uterine function.

    Observation:

    • Two cases presented with severe morbidity following neovagina creation surgery.
    • These cases highlight potential risks associated with uterine preservation attempts.

    Findings:

    • Three potential causes for functional endometrium with vaginal agenesis exist: imperforate hymen, transverse vaginal septum, or complete agenesis with cervical malformation.
    • When imperforate hymen and transverse vaginal septum are excluded, removal of Müllerian structures is proposed as the optimal initial surgical strategy.

    Implications:

    • This approach may reduce surgical morbidity in select patients with vaginal agenesis.
    • Revisiting surgical management strategies for vaginal agenesis is warranted.
