Tick-borne relapsing fever in children

C T Le

    Pediatrics

    Summary

Early diagnosis of tick-borne relapsing fever (TBRF) is crucial. Recognizing specific clues and examining blood smears can prevent costly "fever of unknown origin" work-ups, especially for those vacationing in Western US forests.

    Area of Science:

    • Infectious Diseases
    • Epidemiology
    • Microbiology

    Background:

    • Tick-borne relapsing fever (TBRF) is an infection transmitted by ticks.
    • Diagnosis can be challenging, often presenting as a fever of unknown origin.
    • Increased outdoor recreation in endemic areas raises concern for TBRF.

    Observation:

    • Three cases of TBRF diagnosed in the summer of 1979 are presented.
    • Key diagnostic factors include specific historical clues and peripheral blood smear examination.
    • Clinical manifestations and treatment of TBRF are reviewed.

    Findings:

    • Prompt diagnosis of TBRF is achievable with careful patient history and blood smear analysis.
    • Early identification avoids prolonged and expensive investigations for undiagnosed fevers.
    • TBRF should be considered in patients with nonspecific febrile illness and a history of exposure in Western US forests.

    Implications:

    • Timely diagnosis and treatment of TBRF can improve patient outcomes.
    • Awareness of TBRF is important for clinicians managing febrile illnesses in endemic regions.
    • Public health education regarding tick bite prevention during outdoor activities is recommended.
