    Summary

    This study investigated human leukocyte antigen (HLA) profiles in chronic active hepatitis (CAH). HLA-A9 antigen frequency was elevated in HBsAg-negative CAH patients, while HLA-A2 was lower in this group compared to controls.

    Background:

    • Chronic active hepatitis (CAH) is an inflammatory liver disease with complex etiologies.
    • Human leukocyte antigen (HLA) genes play a crucial role in immune responses and disease susceptibility.
    • Previous studies suggest associations between specific HLA antigens and viral hepatitis.

    Purpose of the Study:

    • To investigate the distribution of HLA antigens in patients with chronic active hepatitis.
    • To compare HLA antigen frequencies between hepatitis B surface antigen (HBsAg)-positive and HBsAg-negative CAH patients.
    • To identify potential HLA associations with CAH and HBsAg status.

    Main Methods:

    • Seventy-five patients with chronic active hepatitis were analyzed for HLA antigen profiles.
    • Patients were categorized into HBsAg-positive (28) and HBsAg-negative (28) groups.
    • HLA antigen frequencies were compared between patient groups and healthy controls.

    Main Results:

    • A moderately increased frequency of HLA-A9 antigen was observed in the overall CAH cohort compared to controls.
    • The HBsAg-negative CAH group showed a significant increase in HLA-A9 frequency.
    • HLA-A2 frequency was notably lower in HBsAg-negative CAH patients compared to HBsAg-positive patients and controls.
    • HLA-A1 and HLA-B8 frequencies were not elevated in any CAH group.
    • HLA-B8 was less frequent in HBsAg-positive CAH patients than in HBsAg-negative CAH patients.

    Conclusions:

    • The HLA-A9 antigen may be associated with susceptibility to HBsAg-negative chronic active hepatitis.
    • Distinct HLA profiles may exist between HBsAg-positive and HBsAg-negative forms of CAH.
    • Further research is warranted to elucidate the immunogenetic mechanisms underlying CAH subtypes.
