    Background:

    Purpose of the Study:

    • To establish reliable criteria for performing laparotomy in patients with acute pelvic fractures.
    • To evaluate the accuracy of existing indications for laparotomy.
    • To identify specific fracture patterns associated with intra-abdominal visceral injury.

    Main Methods:

    • Retrospective review of 224 patients with acute pelvic fractures.
    • Analysis of 44 patients who underwent laparotomy.
    • Correlation of injury mechanisms, fracture types, and intra-abdominal findings.

    Main Results:

    • Bilateral pubic rami fractures were strongly associated with bladder rupture.
    • Clinical signs (abdominal disorder, penetrating wound, shock, evisceration, diminished pulses) and imaging (pyleography, cystography) showed 90% accuracy for correctable intra-abdominal injury.
    • Peritoneal lavage had lower accuracy (57%).

    Conclusions:

    • Established criteria, including specific clinical signs and imaging, enhance accuracy in diagnosing intra-abdominal injuries in pelvic fractures.
    • Further consideration of enlarging hematoma, pelvic bone fragments, persistent bleeding, and rectal/perineal wounds is recommended.
