Summary

Rheumatoid arthritis patients showed significant synovial carcinoembryonic antigen (CEA)-like activity, unlike those with gonarthrosis. This suggests a locally synthesized protein contributes to this activity in rheumatoid arthritis.

Background:

Carcinoembryonic antigen (CEA)-like activity has been observed in various inflammatory conditions.
The presence and significance of CEA-like substances in synovial fluid of rheumatoid arthritis (RA) patients require further investigation.

Purpose of the Study:

To investigate the presence and characteristics of CEA-like activity in the synovial fluid of patients with rheumatoid arthritis and other joint conditions.
To explore the potential source and nature of the CEA-like substance detected.

Main Methods:

Synovial fluid samples were collected from patients diagnosed with rheumatoid arthritis, recurrent arthrosynovitis, and gonarthrosis.
CEA-like activity in synovial samples was assessed using specific assays.
Plasmatic CEA values were measured in parallel.

Main Results:

CEA-like activity was detected in 80% (8/10) of rheumatoid arthritis patients' synovial samples.
Two patients with recurrent arthrosynovitis also showed CEA-like activity.
No CEA-like activity was found in gonarthrosis patients' synovial samples.
Plasmatic CEA levels were normal in non-rheumatoid patients and elevated in only two rheumatoid arthritis cases.

Conclusions:

The findings suggest that CEA-like activity in synovial fluid is a potential biomarker associated with rheumatoid arthritis.
The observed decrease in CEA-like activity with enzymatic proteolysis indicates that the antigenic determinants likely belong to a locally synthesized protein.
Further research is warranted to elucidate the precise role and origin of this protein in rheumatoid arthritis pathogenesis.
