    Background:

    • Mitral valve prolapse (MVP) is associated with cerebral and retinal ischemic events.
    • The role of platelets in the pathogenesis of thromboembolism in MVP remains unclear.

    Purpose of the Study:

    • To investigate the role of platelets in the development of thromboembolism in patients with mitral valve prolapse.
    • To compare platelet function and activity in MVP patients with and without thromboembolic events.

    Main Methods:

    • Studied 29 patients with mitral valve prolapse (MVP) divided into three groups: with thromboembolism, with transient visual obscurations, and without symptoms.
    • Compared platelet coagulant activities, circulating platelet aggregates, and platelet sensitivity to epinephrine with 18 control patients and 38 normal subjects.

    Main Results:

    • MVP patients, particularly those with thromboembolism (Group I) and visual disturbances (Group II), exhibited increased platelet coagulant activities.
    • Group I patients showed a higher proportion of circulating platelet aggregates and reduced platelet sensitivity to epinephrine.
    • Platelet coagulation hyperactivity was observed in 76% of MVP patients (100% in Group I, 75% in Group II) versus 6% in controls.

    Conclusions:

    • Platelets appear to play a significant role in the thromboembolic complications associated with mitral valve prolapse.
    • Increased platelet activity and aggregation may be key mechanisms linking MVP to thromboembolic events.
