    Summary

Urinary diversion procedures are feasible and beneficial for elderly patients over 70, offering palliative care and treatment for bladder cancer with encouraging outcomes. Ureterostomy is a quick option with fewer complications for those with limited life expectancy.

    Area of Science:

    • Urology
    • Geriatric Surgery

    Background:

    • Urinary diversion and cystectomy are established procedures, but their application in elderly patients remains under-explored.
    • Limited data exists on the feasibility and outcomes of urinary diversion in patients over 70 years of age.

    Purpose of the Study:

    • To evaluate the relevance and feasibility of urinary diversion in patients aged 70 and above.
    • To assess the outcomes of various urinary diversion techniques in an elderly cohort.

    Main Methods:

    • Retrospective study of 45 patients over 70 years old who underwent urinary diversion.
    • Procedures included ileal conduit, ureterosigmoidostomy, cutaneous ureterostomy, and transureterocutaneous ureterostomy.
    • Diversions were performed for bladder cancer treatment, palliative care, or benign conditions.

    Main Results:

    • Urinary diversion in elderly patients yielded encouraging results, with a potential life expectancy of up to 9 years.
    • Diversion effectively provided palliation in preterminal disease.
    • Ureterostomy emerged as a rapidly performed procedure with minimal early postoperative complications, suitable for patients with limited life expectancy.

    Conclusions:

    • Urinary diversion is a viable and recommended option for elderly patients, even in cases of advanced or benign disease.
    • The study supports the continuation of recommending urinary diversion for selected elderly patients.
    • Ureterostomy should be seriously considered for elderly patients with limited life expectancy due to its rapid execution and low complication rate.
