    Chronic uncontrolled coeliac disease can lead to rare complications like cutaneous leucocytoclastic vasculitis and nephritis. A strict gluten-free diet resolved these serious conditions, highlighting the importance of celiac disease management.

    • Coeliac disease is an autoimmune disorder triggered by gluten, primarily affecting the small intestine.
    • Uncontrolled coeliac disease can lead to various extra-intestinal manifestations and complications.
    • Cutaneous leucocytoclastic vasculitis is a rare condition characterized by inflammation of small blood vessels in the skin.

    Observation:

    • A 38-year-old female with chronic, uncontrolled coeliac disease presented with cutaneous leucocytoclastic vasculitis.
    • Renal biopsy revealed mesangial nephritis with haematuria and proteinuria.
    • No other cause for the vasculitis or nephritis was identified besides the coeliac disease.

    Findings:

    • Immune complex deposition, potentially triggered by antigens crossing the abnormal small bowel mucosa, is speculated as the cause of vasculitis and nephritis.
