    Summary

    Newborns show elevated thyroglobulin (Tg) levels in the first days of life, independent of thyroid hormones (T3, T4) and TSH. This rise may be due to reduced clearance or increased thyroid secretion of Tg.

    Background:

    • Thyroglobulin (Tg) is a key protein in thyroid hormone synthesis.
    • Understanding neonatal thyroid hormone dynamics is crucial for assessing infant health.

    Purpose of the Study:

    • To investigate the pattern of serum thyroglobulin (Tg) concentrations in newborns during the first 96 hours of life.
    • To examine the relationship between Tg levels and other thyroid function markers (TSH, T3, T4) in neonates.

    Main Methods:

    • Blood samples collected from 16 mothers and their newborns at delivery and at multiple time points up to 96 hours postpartum.
    • Serum concentrations of thyroglobulin (Tg), thyroid-stimulating hormone (TSH), triiodothyronine (T3), and thyroxine (T4) were measured.

    Main Results:

    • Cord blood Tg levels were higher than maternal levels at birth.
    • A significant and sustained increase in serum Tg levels was observed starting from 6 hours after birth and continuing throughout the 96-hour study period.
    • Neonatal Tg levels showed no correlation with serum T3, T4, or TSH concentrations.

    Conclusions:

    • Elevated neonatal thyroglobulin levels in early life are a distinct physiological phenomenon.
    • Possible explanations include reduced metabolic clearance rate (MCR) of Tg or increased thyroidal Tg secretion stimulated by endogenous TSH.
    • Further research is needed to elucidate the precise mechanisms behind this neonatal Tg surge.
