Primary splenic neutropenia--revisited

    Summary

    Splenectomy rapidly normalized neutrophil counts in a patient with chronic neutropenia. This suggests idiopathic neutropenia, not a primary spleen disorder, as the cause.

    Background:

    • Describes a patient with acquired, isolated, chronic, severe neutropenia.
    • Characterized by minimal splenomegaly and bone marrow myeloid hyperplasia.

    Observation:

    • Splenectomy led to a rapid increase in absolute neutrophil count to normal levels within 18 hours post-splenic artery ligation.
    • No other underlying disorders were identified.

    Findings:

    • Tests for antineutrophilic autoantibodies were negative before and after splenectomy.
    • The findings suggest the neutropenia was not caused by splenic dysfunction.

    Implications:

    • This case supports classifying such conditions as 'idiopathic neutropenia' rather than 'primary splenic neutropenia'.
    • The underlying abnormality is likely intrinsic to the neutrophil itself.
