    Summary

    Visual evoked potentials (VEPs) reliably diagnose anterior visual pathway lesions. In occipital lobe lesions, VEPs show abnormalities correlating with visual field defects, aiding diagnosis.

    Background:

    • Pattern reversal visual evoked potentials (VEPs) are established for diagnosing anterior visual pathway lesions.
    • Their utility in posterior visual pathway (occipital lobe) lesions is less consistent.
    • This study investigates VEPs in patients with confirmed occipital lobe lesions.

    Observation:

    • VEPs were recorded in 20 patients diagnosed with occipital lobe lesions.
    • Specific VEP components, notably the P94 wave, were analyzed.
    • Electrode placement focused on scalp areas overlying the occipital lobes.

    Findings:

    • A maximal P94 VEP response was observed over the occipital lobes.
    • This maximal response was contralateral to the hemianoptic visual field defect.
    • A positive correlation was found between unilateral occipital lobe lesions, homonymous visual field loss, and VEP abnormalities.

    Implications:

    • VEPs can be a valuable tool for detecting and localizing occipital lobe lesions.
    • Abnormal VEPs correlate with homonymous visual field defects, enhancing diagnostic accuracy.
    • This supports the use of VEPs in evaluating posterior visual pathway disorders.
