    Background:

    • Multiple sclerosis (MS) is a chronic inflammatory disease primarily affecting the central nervous system (CNS).
    • While CNS demyelination is characteristic, peripheral nervous system (PNS) involvement is less understood.

    Observation:

    • A unique case of MS presented with recent demyelinated plaques in the CNS.
    • Simultaneously, inflammatory demyelination and remyelination were observed in the peripheral (Schwann cell) portions of spinal roots.

    Findings:

    • Peripheral nervous system lesions included inflammation, primary segmental demyelination, and myelin stripping.
    • Endoneurial macrophages containing lipid debris and evidence of remyelination were noted in the PNS lesions.
    • This represents the first documented instance of concurrent acute inflammatory demyelination in both the CNS and PNS in MS.
