Candidiasis in the obstructed esophagus

W B Gefter, I Laufer, S Edell

    Radiology
    |January 1, 1981
    PubMed
    Summary

Esophageal candidiasis can occur with functional or mechanical obstruction, even without typical risk factors. Esophageal stasis, regardless of the cause, may lead to this fungal infection.

    Area of Science:

    • Gastroenterology
    • Mycology
    • Medical Imaging

    Background:

    • Esophageal candidiasis typically presents in immunocompromised individuals or those with specific predisposing conditions.
    • Functional or mechanical obstruction of the esophagus is not commonly recognized as a direct risk factor for candidiasis.

    Observation:

    • This study details four cases where esophageal candidiasis complicated existing esophageal obstruction.
    • Obstruction causes included achalasia, scleroderma, and post-surgical changes (fundoplication).
    • Patients lacked the usual risk factors for esophageal candidiasis.

    Findings:

    • Barium studies revealed diverse mucosal abnormalities, including plaque-like defects and nodularity.
    • These findings mimic ingested debris, necessitating careful radiological interpretation.
    • Esophageal stasis, irrespective of its origin, was identified as a potential precursor.

    Implications:

    • Esophageal stasis should be considered a significant risk factor for candidiasis.
    • Clinicians should suspect candidiasis in patients with esophageal obstruction and altered mucosal appearance.
    • Further research into the pathogenesis of candidiasis in stasis conditions is warranted.
