Sublingual nitroglycerin safely and effectively dilates the bile duct for endoscopic stone removal. This minimally invasive procedure achieved a 93% success rate in patients, preserving sphincter function and potentially preventing complications.

Area of Science:

  • Gastroenterology
  • Endoscopic procedures
  • Biliary tract interventions

Background:

  • Common bile duct stones often require endoscopic retrograde cholangiopancreatography (ERCP) for removal.
  • Endoscopic sphincterotomy (EST) is a common procedure but can lead to complications.
  • Minimally invasive methods to facilitate stone removal while preserving sphincter function are needed.

Observation:

  • Sublingual nitroglycerin was used to achieve medical sphincter dilation prior to stone removal.
  • A low dose of nitroglycerin (0.3-0.6 mg) provided sufficient dilation for cannulation.
  • Endoscopic stone removal was performed in 15 patients using a basket catheter.

Findings:

  • Complete stone removal was achieved in 14 out of 15 patients (93.3% success rate).

Related Experiment Videos

  • A mechanical lithotriptor was used in one case due to potential impaction.
  • Transient hypotension occurred in one patient but was medically managed without issue.
  • Implications:

    • This technique offers a safe, easy, and effective alternative for common bile duct stone removal.
    • Preserving sphincter function may help avoid complications like acute cholecystitis post-sphincterotomy.
    • Further large-scale, long-term studies are warranted to fully assess safety and efficacy.
