    Area of Science:

    • Clinical Nutrition
    • Hematology
    • Gastroenterology

    Background:

    • A 51-year-old obese woman underwent laparoscopic cholecystectomy.
    • Complications included fever, malaise, and a discovered liver abscess.

    Observation:

    • Postoperative day 52, the patient developed a magenta, sore tongue, altered taste, pallor, weight loss, diarrhea, and poor appetite.
    • Dermatology suggested nutrition-related tongue abnormalities; hematology indicated possible folate deficiency.

    Findings:

    • The patient had a history of a folate-deficient diet and long-term estrogen therapy.
    • Prolonged hospitalization involved fever and multiple antibiotic treatments.
    • Nutritional status improved significantly within a month of initiating nutritional support and vitamin/mineral supplements.

    Implications:

    • Poor diet, estrogen therapy, surgical stress, and infection may have contributed to folate deficiency.
    • Early nutritional intervention is vital for patients with complex medical conditions and potential nutrient deficiencies.
    • This case underscores the importance of considering nutritional status in prolonged hospitalizations and complex surgical recovery.
