Area of Science:

  • Infectious Diseases
  • Pulmonology
  • Endocrinology

Background:

  • Tuberculosis (TB) primarily affects the lungs but can manifest in extrapulmonary sites.
  • Diagnosis of extrapulmonary TB can be challenging due to vague presenting symptoms.

Observation:

  • Three patients (two women, one man) with prolonged residence in the Netherlands presented with non-specific symptoms.
  • Diagnostic workup revealed extrapulmonary tuberculosis in all three individuals.

Findings:

  • Diagnoses included lumbar spine tuberculosis with psoas abscess, tuberculous peritonitis, and adrenal tuberculosis leading to Addison's disease in a patient with co-existing open pulmonary tuberculosis.
  • All patients demonstrated complete recovery following a four-drug tuberculostatic regimen (isoniazid, streptomycin, pyrazinamide, and rifampicin).

Related Experiment Videos

Implications:

  • Highlights the importance of considering extrapulmonary TB in patients with vague symptoms, even in low-prevalence settings.
  • Demonstrates the efficacy of standard multi-drug therapy for treating complex extrapulmonary tuberculosis presentations.
  • Underscores the potential for TB to affect multiple organ systems, necessitating a comprehensive diagnostic approach.
