Comparison of histologic stains in the diagnosis of Pneumocystis carinii
1Veterans Affairs Medical Center Long Beach, CA 90822, USA.
Objective:
To evaluate commonly used histologic stains used to detect Pneumocystis, including Diff-Quik (DQ), Grocott's Gomori methenamine silver (GMS), Papanicolaou, and hematoxylin and eosin (H&E).
Study Design:
The study compared the ability of these stains to detect alveolar casts.
Results:
Although there was a slightly higher average number of casts detected with DQ, the difference between this stain and GMS was not significant. Both Papanicolaou stain and H&E showed significantly lower averages. Some differences in H&E may be related to differences in preparation.
Conclusion:
Similar results were obtained with DQ and GMS, but DQ offered the advantage of ease of preparation, rapidity and cost-effectiveness.
Insights
Diff-Quik (DQ) and Grocott's Gomori methenamine silver (GMS) stains effectively detect Pneumocystis alveolar casts. DQ offers advantages in preparation, speed, and cost, making it a practical choice for diagnosing Pneumocystis pneumonia.
Area of Science:
- Histopathology
- Medical Diagnostics
- Microbiology
Background:
- Accurate detection of Pneumocystis is crucial for diagnosing Pneumocystis pneumonia.
- Histologic stains are commonly used to identify Pneumocystis organisms and associated alveolar casts.
- Evaluating the efficacy of different stains is essential for optimizing diagnostic workflows.
Purpose of the Study:
- To compare the diagnostic performance of commonly used histologic stains for detecting Pneumocystis.
- To assess the ability of Diff-Quik (DQ), Grocott's Gomori methenamine silver (GMS), Papanicolaou, and hematoxylin and eosin (H&E) stains in identifying alveolar casts.
Main Methods:
- A comparative study was conducted to evaluate four distinct histologic stains.
- The primary outcome measured was the ability of each stain to detect alveolar casts indicative of Pneumocystis infection.
Main Results:
- Diff-Quik (DQ) and Grocott's Gomori methenamine silver (GMS) demonstrated comparable efficacy in detecting alveolar casts.
- Papanicolaou stain and hematoxylin and eosin (H&E) showed significantly lower detection rates for alveolar casts.
- Variations in H&E results may be attributed to differences in tissue preparation methods.
Conclusions:
- Diff-Quik (DQ) and GMS stains yield similar results for detecting Pneumocystis alveolar casts.
- DQ stain presents practical advantages including ease of preparation, rapid staining, and cost-effectiveness.
- DQ is a viable and efficient alternative for the histologic diagnosis of Pneumocystis pneumonia.
