Summary

Lower extremity amputation is associated with deep vein thrombosis (DVT) in 11% of patients, primarily at or above the popliteal vein. This finding highlights the need for DVT management strategies in amputation patients.

Area of Science:

  • Vascular Surgery
  • Thrombosis Research
  • Amputation Medicine

Background:

  • Patients undergoing lower extremity amputation are often perceived to be at high risk for deep vein thrombosis (DVT).
  • Limited data exist to confirm the actual incidence of DVT in this patient population.
  • Prospective documentation is needed to guide clinical practice.

Purpose of the Study:

  • To prospectively determine the incidence of deep vein thrombosis (DVT) complicating lower extremity amputation.
  • To evaluate risk factors and patterns of DVT in patients undergoing amputation.

Main Methods:

  • A prospective study involving 72 patients (71 men, 1 woman; mean age 68 years) undergoing major lower extremity amputation.
  • Perioperative duplex scanning was utilized to evaluate for DVT in 31 above-knee and 41 below-knee amputations over 28 months.

Main Results:

  • Deep vein thrombosis (DVT) was detected in 12.5% (9/72) of patients.
  • A history of venous disease was a significant risk factor for DVT development (p = 0.02).
  • Thrombi were predominantly located at or proximal to the popliteal vein.

Conclusions:

  • Lower extremity amputation is associated with a DVT incidence of approximately 11%, primarily involving the popliteal vein.
  • Documenting DVT prevalence is crucial for developing effective prevention, diagnosis, and treatment strategies.
  • Early identification and management with anticoagulation appear to prevent clinical symptoms of pulmonary embolism.
