A rare complete persistent sciatic artery was found in an asymptomatic patient with aortoiliac aneurysms. Surgical repair without peripheral reconstruction proved successful, avoiding ischemic complications.

Background:

  • Persistent sciatic artery (PSA) is a rare congenital anomaly.
  • It can be associated with aneurysmal dilatation of the aorta and iliac arteries.
  • Asymptomatic presentation is uncommon.

Observation:

  • A case of a complete persistent sciatic artery was identified in an asymptomatic individual.
  • The patient presented with multiple aortoiliac aneurysms.
  • The anomaly was incidentally discovered during imaging for the aneurysms.

Findings:

  • The patient underwent endoaneurysmorraphy and ligation of the persistent sciatic artery at its origin.
  • The surgical intervention was successful, with no subsequent ischemic complications.
  • Peripheral vascular reconstruction, typically advised in such cases, was deemed unnecessary and omitted.

Implications:

  • This case suggests that peripheral vascular reconstruction may not always be necessary in patients with persistent sciatic artery and aortoiliac aneurysms.
  • Conservative management or tailored surgical approaches might be sufficient.
  • Further research is warranted to establish definitive guidelines for managing this rare condition.
