Background:

  • Benign prostatic hyperplasia (BPH) affects urinary bladder (UB) function.
  • Assessing UB electrical activity can aid in BPH diagnosis and staging.

Purpose of the Study:

  • To investigate the utility of electrovesicogram (EVG) in differentiating compensated and decompensated BPH stages.
  • To characterize the electric activity patterns of the UB in healthy volunteers and BPH patients.

Main Methods:

  • EVG recordings were obtained from 26 BPH patients (10 compensated, 16 decompensated) and 10 healthy controls.
  • Three electrodes were placed on the hypogastric area; recordings were taken from full, empty, and post-voiding UB states.
  • Pacesetter potentials (PPs) were analyzed for amplitude, frequency, and velocity.

Main Results:

  • Healthy volunteers exhibited reproducible triphasic PPs in the UB.
  • Compensated BPH patients showed significantly higher PP frequency, amplitude, and velocity ('tachyvesica') compared to controls (p < .01).
  • Decompensated BPH patients displayed a bradyarrhythmic pattern in a full UB and a silent pattern in an empty UB.

Conclusions:

  • EVG can differentiate between compensated and decompensated BPH.
  • The EVG technique is simple, noninvasive, and offers a valuable alternative to other investigative methods for BPH assessment.
