Background:

Purpose of the Study:

  • To extensively define the pattern of epithelial keratin expression in the normal anal canal.
  • To investigate alterations in intermediate filaments in pathological anal epithelial lesions.

  • Immunohistological investigation of 35 frozen haemorrhoid tissue specimens.
  • Analysis of 14 individual keratins (K) using 17 monoclonal antibodies.

Main Results:

  • Perianal skin showed specific basal and suprabasal keratin expressions (K5, 14, 17, 10, 1, 16).
  • Anal squamous epithelium exhibited basal K5, 17 and K4, 13, 16 positivity, with sporadic K1, 10.
  • The anal transitional zone (ATZ) and anal glands/ducts showed K19 and simple epithelial keratins (K7, 8, 18, 19).

Conclusions:

  • A detailed map of keratin expression in the normal anal canal was established.
  • Heterogeneous keratinocyte keratin expression was observed, including anomalous K4 and K13 expression in the ATZ.
  • This data provides a baseline for diagnosing anal epithelial pathologies.
