Summary

Graves' disease and toxic nodular goiter elevate soluble adhesion molecules. While some levels normalize with treatment, others persist, suggesting both thyroid hormones and autoimmune factors contribute to these changes.

Background:

  • Thyroid dysfunction, including Graves' disease (GD) and toxic nodular goiter (TNG), is associated with systemic inflammation.
  • Soluble adhesion molecules play a role in inflammatory processes and vascular cell interactions.

Purpose of the Study:

  • To investigate serum levels of various soluble adhesion molecules and cytokines in patients with GD and TNG.
  • To assess the impact of methimazole treatment on these markers.
  • To explore correlations between adhesion molecules, cytokines, thyroid autoantibodies, and thyroid hormones.

Main Methods:

  • Serum concentrations of soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), soluble E-selectin (sE-selectin), soluble P-selectin, soluble L-selectin (sL-selectin), tumor necrosis factor-alpha, and interleukin-6 were measured.

  • Measurements were taken in patients with GD (n=33), TNG (n=34), and healthy controls (n=36).
  • Follow-up measurements were performed after 8 weeks of methimazole therapy.
  • Main Results:

    • sICAM-1, sVCAM-1, sE-selectin, and sL-selectin were elevated in GD and TNG patients before treatment.
    • sVCAM-1 and sE-selectin normalized after 8 weeks of methimazole, while sICAM-1 and sL-selectin remained elevated.
    • Hormone levels normalized within 2 weeks, preceding the normalization of adhesion molecules.
    • No significant differences in soluble P-selectin, TNF-alpha, or IL-6 were observed among groups.
    • sVCAM-1 and sICAM-1 levels correlated with TSH receptor antibodies and thyroid peroxidase antibodies.

    Conclusions:

    • Elevated soluble adhesion molecules are present in both Graves' disease and toxic nodular goiter.
    • The autoimmune process and thyroid hormone action likely contribute to increased soluble adhesion molecule levels.
    • Treatment with methimazole partially reverses these elevations, but some markers remain elevated, indicating ongoing inflammatory processes.
