Summary

Delirium can occur in elderly patients taking mianserin, a drug for depression. Despite its low anticholinergic activity, mianserin may cause more cognitive side effects than previously thought.

Area of Science:

  • Geriatric Medicine
  • Psychopharmacology
  • Neuroscience

Background:

  • Delirium is a neuropsychiatric condition that can be triggered by medications in vulnerable individuals.
  • Tricyclic antidepressants are known to cause delirium due to their anticholinergic effects.
  • Mianserin, with low anticholinergic properties, is often prescribed for depression in the elderly.

Purpose of the Study:

  • To investigate the potential for mianserin to induce delirium in elderly patients.
  • To assess the incidence of organo-mental side effects associated with mianserin use in this population.

Main Methods:

  • Case series reporting on five elderly patients who developed delirium while treated with mianserin.
  • Review of clinical data and medication history for each patient.

Main Results:

  • Five cases of delirium were observed in elderly patients treated with standard doses of mianserin.
  • The findings suggest a potentially higher incidence of cognitive side effects with mianserin than previously documented.

Conclusions:

  • Mianserin, despite its favorable anticholinergic profile, may precipitate delirium in vulnerable elderly patients.
  • Further research is warranted to fully understand the risk-benefit profile of mianserin regarding cognitive side effects in geriatric populations.
