Area of Science:

  • Nephrology
  • Glomerular Diseases
  • Renal Pathology

Background:

  • Glomerular diseases present with diverse clinical and pathological features.
  • Collapsing glomerulopathy is a severe form of kidney disease.
  • Early diagnosis and understanding of its variants are crucial for patient outcomes.

Observation:

  • A 30-year-old Black female presented with rapidly declining renal function.
  • Renal biopsy revealed diffuse glomerular capillary collapse.
  • Extracapillary hypercellularity was noted without urinary space obliteration or parietal cell proliferation.

Findings:

  • The case exhibited electron-dense deposits and immune complex deposition (IgG and C3).
  • Minimal tubular atrophy and interstitial fibrosis were observed.
  • Histopathology confirmed a collapsing glomerulopathy with immunoglobulin and electron-dense deposits.

Implications:

  • This case highlights an unusual presentation of collapsing glomerulopathy in a young demographic.
  • Understanding specific pathological features aids in differentiating from other glomerular diseases.
  • Further research into the pathogenesis and treatment of such variants is warranted.
