Summary

Angiotensin-converting enzyme inhibitors like Captopril can cause interstitial pulmonary infiltrates and dyspnea. Discontinuation of the drug led to rapid symptom resolution and radiographic improvement.

Background:

  • A 60-year-old woman with severe left-ventricular dysfunction and mitral regurgitation was treated with Captopril.
  • Initial treatment showed improvement in left ventricular function and pulmonary capillary wedge pressure.

Observation:

  • Despite cardiac improvements, the patient developed worsening exertional dyspnea and a severe dry cough.
  • Chest imaging revealed interstitial infiltrates, but pulmonary function tests were normal.

Findings:

  • Discontinuation of Captopril resulted in rapid resolution of dyspnea and cough.
  • Interstitial infiltrates significantly regressed after Captopril withdrawal.

Implications:

  • This case highlights angiotensin-converting enzyme inhibitors as a potential cause of drug-induced interstitial lung disease.
