Alcoholic hepatitis with and without alcoholic hyaline

S Nakamura, Y Takezawa, T Sato

The Tohoku Journal of Experimental Medicine
    |December 1, 1977

    Summary

Alcoholic hepatitis with alcoholic hyaline showed more fatty liver changes. Clinical differences were minimal, but hepatic venous pressure was elevated, suggesting portal hypertension in severe alcoholic hepatitis cases.

    Area of Science:

    • Hepatology
    • Gastroenterology
    • Internal Medicine

    Background:

    • Alcoholic hepatitis is a severe liver disease.
    • Alcoholic hyaline is a histological marker found in liver cells.
    • Understanding its clinical significance is crucial for patient management.

    Purpose of the Study:

    • To compare clinical and laboratory findings in alcoholic hepatitis with and without alcoholic hyaline.
    • To investigate the association between alcoholic hyaline and liver histology, and portal hemodynamics.

    Main Methods:

    • Retrospective analysis of 16 cases with alcoholic hyaline (Group I) and 13 cases without (Group II).
    • Review of clinical data, laboratory tests, liver biopsy findings, and hepatic venous pressure measurements.
    • Hepatic venography was performed in a subset of patients.

    Main Results:

    • Both groups predominantly affected individuals in their fifth decade, with heavy alcohol consumption.
    • Fever and elevated erythrocyte sedimentation rate were more common in Group I.
    • Fatty changes were more frequent in Group I; elevated hepatic venous pressure indicated portal hypertension, particularly in cirrhotic patients.

    Conclusions:

    • Alcoholic hyaline presence correlates with more pronounced fatty liver changes.
    • While clinical presentations are similar, hepatic venous pressure elevation suggests potential for portal hypertension in alcoholic hepatitis patients with alcoholic hyaline.
