Summary

Undiagnosed coronal leakage from faulty fillings and decay caused endodontic treatment failure. Replacing fillings and removing decay successfully resolved symptoms and completed root canal therapy.

Area of Science:

  • Dentistry
  • Endodontics
  • Restorative Dentistry

Background:

  • Endodontic treatment aims to save teeth by removing infection from the root canal system.
  • Coronal leakage, the ingress of bacteria or fluids from the mouth into the root canal system, is a significant cause of endodontic failure.
  • Deficient restorations and secondary caries are common sources of coronal leakage.

Observation:

  • A case of persistent symptoms and endodontic treatment failure was reported.
  • The failure was attributed to undiagnosed coronal leakage originating from deficient composite resin fillings and secondary caries.
  • Repeated endodontic cleaning and dressing procedures failed to resolve contamination and symptoms.

Findings:

  • Diagnosis revealed coronal leakage as the primary cause of endodontic failure.
  • The deficient composite resin fillings and underlying secondary caries were identified as the sources of leakage.
  • Successful treatment involved replacing the faulty restorations and eliminating the caries.

Implications:

  • This case highlights the critical importance of diagnosing and managing coronal leakage in endodontic treatment.
  • Proper diagnosis and timely intervention, including restorative work, are essential for successful endodontic outcomes.
  • Preventing coronal leakage through high-quality restorations is key to long-term endodontic success.
