Summary

Background:

  • Vasoactive intestinal peptide (VIP) receptors are overexpressed in certain neuroendocrine tumors.
  • 123I-labeled peptides offer potential for diagnostic imaging.

Purpose of the Study:

  • To evaluate the biodistribution, safety, and absorbed radiation dose of 123I-VIP in patients with intestinal adenocarcinomas or endocrine tumors.
  • To assess the utility of 123I-VIP as a potential imaging agent for VIP receptor-positive tumors.

Main Methods:

  • High-performance liquid chromatography (HPLC) was used to purify 123I-VIP.
  • 18 patients received intravenous 123I-VIP (172 +/- 17 MBq).
  • Sequential imaging and dosimetry calculations were performed using gamma camera data, urine, feces, and blood activity measurements.

Main Results:

  • The lung was the primary site of 123I-VIP uptake, with peak activity at 40% +/- 7% at 0.7 hr postinjection.
  • Radioactivity was primarily excreted via urine, with 93% +/- 8% excreted within 24 hours.
  • Highest absorbed doses were to the urinary bladder (77 muGy/MBq) and thyroid (104 muGy/MBq), with an effective dose of 28 muSv/MBq.

Conclusions:

  • HPLC-purified 123I-VIP demonstrates favorable dosimetry and safety profiles.
  • 123I-VIP is a promising peptide tracer for the localization of tumors expressing VIP receptors.
