Okinawan centenarians show increased dementia and disability, with significant declines in Activities of Daily Living (ADL) scores observed between the 1970s and 1990s. Declines were more pronounced in institutionalized individuals, with physical function decreasing more in institutions and cognitive function declining more in home settings.
