Area of Science:

  • Surgical Innovation
  • Obstetric Surgery
  • Gastrointestinal Surgery

Background:

  • Cholecystectomy is a common non-obstetric surgery during pregnancy.
  • Laparoscopic cholecystectomy (LC) was historically contraindicated in gravid patients.
  • Recent successful cases suggest LC may be viable in pregnancy.

Purpose of the Study:

  • To review the safety and outcomes of laparoscopic cholecystectomy (LC) performed during pregnancy.
  • To compare personal experience with published literature on LC in gravid patients.

Main Methods:

  • A review of five LC cases performed during pregnancy between May 1990 and November 1994.
  • A MEDLINE search to retrieve 41 additional published cases of LC during pregnancy.

Main Results:

  • In the author's series, five LCs were performed across the second and third trimesters with no maternal or fetal complications.
  • Review of 41 published cases revealed no fetal loss or maternal morbidity across all trimesters (3-31 weeks).
  • Cholangiography was performed in 15 cases, and tocolytic agents in five of 46 patients.

Conclusions:

  • Laparoscopic cholecystectomy (LC) appears to be a safe procedure for pregnant patients.
  • Skilled laparoscopic surgeons can safely perform LC during pregnancy.
  • Further clinical data is needed to solidify these findings.
