Summary

Primary drug resistance in tuberculosis patients is high, with most M. tuberculosis strains resistant to Streptomycin and Ethambutol. Non-tuberculous mycobacteria showed resistance to common TB drugs.

Background:

  • Tuberculosis (TB) remains a significant global health challenge.
  • Drug resistance in Mycobacterium tuberculosis complicates treatment and control efforts.
  • Understanding primary drug resistance patterns is crucial for effective TB management.

Purpose of the Study:

  • To investigate the primary drug resistance patterns of tubercle bacilli.
  • To analyze resistance in both Mycobacterium tuberculosis and non-tuberculous mycobacteria (MOTT) species.
  • To assess resistance to four key anti-tubercular drugs: Isoniazid, Rifampicin, Streptomycin, and Ethambutol.

Main Methods:

  • Sputum samples from 961 suspected TB patients were collected.
  • Ziehl-Neelsen staining identified Acid Fast Bacilli (AFB).

  • Microscopically positive AFB cases (105) were cultured on Lowenstein-Jensen media, with 91 identified as M. tuberculosis and 9 as MOTT species.
  • Main Results:

    • High sensitivity of M. tuberculosis to Isoniazid (97.80%), Streptomycin (100%), and Ethambutol (100%).
    • Lower sensitivity of M. tuberculosis to Rifampicin (80.21%).
    • MOTT species showed high resistance to Isoniazid and Rifampicin, but sensitivity to Streptomycin (44.44%) and Ethambutol (77.78%).
    • Overall, 27 isolates (18 M. tuberculosis, 9 MOTT) exhibited resistance to at least one drug, with significant multi-drug resistance observed in MOTT species.

    Conclusions:

    • Primary drug resistance in pulmonary tuberculosis patients in Dhaka is a concern.
    • M. tuberculosis strains showed high sensitivity to most tested drugs, except Rifampicin.
    • MOTT species exhibited substantial resistance to Isoniazid and Rifampicin, highlighting the need for accurate species identification and tailored treatment strategies.
