Summary

Gallbladder perforation into the liver is a rare cause of liver abscesses. Imaging and percutaneous drainage can diagnose and treat this condition, especially when the gallbladder is intrahepatic.

Background:

  • Gallbladder perforation leading to cholecystohepatic communication is an uncommon etiology for liver abscess formation.
  • This study investigates four cases of liver abscess attributed to gallbladder perforation.

Purpose of the Study:

  • To report on the diagnostic and therapeutic imaging modalities for liver abscesses caused by gallbladder perforation.
  • To highlight the association between intrahepatic gallbladder and this rare condition.

  • Retrospective review of 39 patients undergoing percutaneous liver abscess drainage.
  • Identification of four patients with hepatic abscess secondary to gallbladder perforation.
  • Utilized sonography and computed tomography (CT) for diagnosis and abscessograms for treatment confirmation.

Main Results:

  • Sonography revealed hypoechoic liver lesions adjacent to the gallbladder in all four patients.
  • CT demonstrated corresponding hypodense liver areas.
  • Percutaneous drainage and abscessogram confirmed gallbladder communication with the abscess cavity in all cases.
  • Two patients underwent cholecystectomy, confirming gallbladder fundus perforation and intrahepatic positioning.

Conclusions:

  • Gallbladder perforation is a rare cause of pyogenic liver abscess.
  • The incidence may be higher when the gallbladder is partially or totally intrahepatic, as suggested by surgical cases and literature review.
