Summary

Malignant thymoma, a rare anterior mediastinum tumor, can cause dyspnea and pericardial tamponade in elderly patients. This case highlights a previously undescribed presentation of symptomatic pericardial effusion due to thymoma.

Area of Science:

  • Oncology
  • Thoracic Surgery
  • Cardiology

Background:

  • Thymomas are neoplasms originating in the thymus gland, typically located in the anterior mediastinum.
  • Most thymomas are asymptomatic and discovered incidentally on chest imaging.
  • The incidence peaks in adults aged 45-50 years.

Observation:

  • This report details a case of an elderly woman presenting with dyspnea.
  • The patient's symptoms were attributed to a malignant thymoma.
  • A massive pericardial effusion leading to symptomatic pericardial tamponade was observed.

Findings:

  • Malignant thymoma can present with severe symptoms, including dyspnea.
  • Symptomatic pericardial tamponade secondary to a massive pericardial effusion caused by thymoma is a rare clinical manifestation.
  • This presentation is unusual, particularly in an elderly patient.

Implications:

  • Highlights the importance of considering thymoma in the differential diagnosis of unexplained dyspnea and pericardial effusion, even in elderly patients.
  • Suggests that thymomas, even when malignant, may present with advanced complications.
  • Underscores the need for thorough diagnostic workup in patients with mediastinal masses and cardiac symptoms.
