    Summary

    This study evaluated an extended transverse neck incision for head and neck cancer surgery. While complications like fistula and infection occurred, the incision provided good carotid artery protection and surgical exposure.

    Background:

    Purpose of the Study:

    • To assess the efficacy and safety of an extended single transverse neck incision for composite resections in head and neck cancer patients.

    • An extended single transverse neck incision was utilized in an unselected group of 40 patients.
    • Patients underwent composite resection for carcinoma of the head and neck.
    • Outcomes including fistula, wound infection, flap necrosis, carotid artery exposure/rupture, cosmetic deformity, and reconstructive feasibility were recorded.

    Main Results:

    • 21 out of 40 patients (52.5%) developed fistula and wound infection.
    • Carotid artery exposure due to flap necrosis occurred in 2 patients (5%).
    • Carotid artery rupture occurred in 2 patients (5%).
    • The incision provided a viable flap with excellent blood supply, adequate surgical exposure, and minimal cosmetic deformity.
    • The approach did not interfere with primary or secondary reconstruction.

    Conclusions:

    • The extended single transverse neck incision offers a viable flap with excellent blood supply for head and neck cancer surgery.
    • Despite a notable rate of fistula and infection, this approach generally provides good carotid artery protection.
    • The technique facilitates adequate surgical exposure and reconstruction while minimizing cosmetic deformity.
