Central nervous system (CNS) involvement is rare in Schistosoma mansoni infections. This report details the first imported case of cerebral schistosomiasis in Korea, caused by S. mansoni.

Area of Science:

  • Neuroparasitology
  • Infectious Diseases
  • Neurology

Background:

  • Chronic schistosomiasis can affect the central nervous system (CNS).
  • While Schistosoma japonicum is a frequent cause, CNS involvement by Schistosoma mansoni is rare.
  • S. mansoni can reach the CNS via egg embolization or adult worm migration.

Observation:

  • A case of cerebral schistosomiasis in a 40-year-old man with a history of working in Yemen is presented.
  • Cerebral involvement was caused by Schistosoma mansoni.

Findings:

  • Biopsies of the cerebellar vermis and fourth ventricle roof revealed granulomatous inflammation due to Schistosoma eggs.
  • Schistosoma mansoni was confirmed via stool examination and ELISA using serum and cerebrospinal fluid (CSF).

Implications:

  • This case represents the first imported instance of cerebral schistosomiasis due to Schistosoma mansoni in Korea.
  • Highlights the importance of considering schistosomiasis in the differential diagnosis of neurological disorders in travelers returning from endemic areas.
