Summary

Mutations in the hepatitis B virus (HBV) core gene are common in chronic infections. These genetic changes, particularly near immune sites, correlate with disease activity and hepatitis B e antigen (HBeAg) clearance.

Purpose of the Study:

  • To determine the frequency and location of naturally occurring HBV core gene mutations in Chinese patients with chronic HBV infection.
  • To investigate the association between core gene mutations and clinical parameters like disease activity and hepatitis B e antigen (HBeAg) status.

Main Methods:

  • Sera from 69 Chinese patients with chronic HBV infection were analyzed.
  • Direct sequencing of polymerase chain reaction amplified HBV DNA was used to identify mutations.
  • Multiple regression analysis was employed to assess associations between mutations and clinical factors.

Main Results:

  • Nearly all patients (69/69) had nucleotide changes, with 64% exhibiting amino acid changes (mean 3.7).
  • Core gene mutations were significantly associated with precore stop-codon mutation, HBeAg negativity, and active liver disease.
  • Higher mutation rates were observed during HBeAg clearance and in HBeAg-negative patients with active disease, predominantly in the middle of the core gene near T-cell epitopes.

Conclusions:

  • Naturally occurring mutations in the HBV core gene are frequent in chronic infections.
  • These mutations are associated with disease activity and occur most often around the time of HBeAg clearance.
