Summary

Quinidine, an antiarrhythmic drug, can rarely cause rheumatologic issues like drug-induced lupus or polymyalgia rheumatica. Early identification and stopping quinidine quickly resolves these symptoms.

Area of Science:

  • Rheumatology
  • Cardiology
  • Pharmacology

Background:

  • Quinidine is a widely prescribed antiarrhythmic medication.
  • While effective, quinidine is rarely linked to rheumatologic adverse effects.
  • Reported cases include quinidine-induced lupus and polymyalgia rheumatica-like illness.

Purpose of the Study:

  • To report two cases of quinidine-induced rheumatologic conditions: drug-induced lupus and polymyalgia rheumatica-like illness.
  • To review the existing English literature on quinidine's rheumatologic toxicity.
  • To emphasize the importance of recognizing and managing quinidine-associated rheumatologic symptoms.

Main Methods:

  • Case presentation of two patients with quinidine-induced rheumatologic conditions.
  • Comprehensive literature review of quinidine-associated rheumatologic toxicity.
  • Analysis of clinical presentation, diagnosis, and treatment outcomes.

Main Results:

  • One case presented as quinidine drug-induced lupus.
  • The second case exhibited a polymyalgia rheumatica-like illness.
  • Literature review confirmed various rheumatologic toxicities associated with quinidine use.

Conclusions:

  • Quinidine can induce a range of rheumatologic conditions, including lupus and polymyalgia rheumatica-like syndromes.
  • Prompt diagnosis and discontinuation of quinidine are crucial for symptom resolution.
  • Awareness of these potential side effects is vital for clinicians managing patients on quinidine.
