Purpose of the Study:

  • To determine long-term survival rates (> 3 months) in patients with gram-negative sepsis.
  • To develop predictive models for long-term sepsis survival.
  • To assess the health and functional status of sepsis survivors.

Main Methods:

  • A cohort of 100 patients with suspected gram-negative sepsis were followed for a median of 7667 patient-months.
  • Cox proportional hazard regression models were used to identify predictors of long-term survival.
  • Health and functional status were measured using validated scoring systems and compared to general population norms.

Main Results:

  • Severity of underlying illness (McCabe and Jackson criteria) and vasopressor use were significant predictors of mortality.
  • Adult respiratory distress syndrome and comorbid illnesses also predicted poor long-term survival.
  • Sepsis survivors reported increased physical dysfunction and poorer general health perception, but improved emotional health perception.

Conclusions:

  • Underlying illness severity and vasopressor use at the onset of sepsis strongly predict both short- and long-term survival.
  • The McCabe and Jackson severity scoring system is validated for predicting long-term sepsis survival.
  • Physical dysfunction and diminished general health perception are common sequelae of sepsis, necessitating continued patient support.
