Summary

Ectatic vertebral and basilar arteries can compress the VIIIth nerve or brainstem, causing symptoms like vertigo and hearing loss. These vascular compressions mimic tumors but may be treatable with surgical decompression.

Background:

  • VIIIth nerve and brainstem disturbances present diagnostic challenges.
  • Potential causes include tumors, infarcts, bleedings, and microvascular loops.
  • Ectatic arteries are increasingly recognized as a cause of neurological symptoms.

Observation:

  • A comprehensive test battery including audiology, electronystagmography, rotatory testing, and dynamic posturography revealed no uniform pattern.
  • Both VIIIth nerve and central nervous system (CNS) signs were observed, along with trigeminal neuralgia and hemifacial spasm.
  • CT, NMR, and angiography identified ectatic vertebral and/or basilar arteries as the common finding in these patients.

Findings:

  • The size and position of ectatic vertebral and/or basilar arteries indicated compression of the VIIIth nerve or brainstem as the cause of neurotological symptoms.

Related Experiment Videos

  • A review of 300 NMR investigations found no such macrovessels in patients without neurotological indications, suggesting a specific link.

    • Implications:

    • Ectatic vessels can cause symptoms mimicking pontine angle tumors, Meniere's disease, and other inner ear or balance disorders.
    • While arterial loops may suggest microvascular surgery, large ectatic arteries present surgical decompression options with substantial risks.
