Endothelial cells show early ultrastructural changes in patients with ruptured abdominal aortic aneurysms. These findings in endothelial cells may contribute to coagulopathy and organ failure after aneurysm rupture.

Background:

  • Endothelial cell activation is an early response to hypovolemic shock.
  • This activation may play a role in coagulopathy and multiple organ failure following aortic aneurysm rupture.

Purpose of the Study:

  • To investigate ultrastructural changes in endothelial cells of patients with ruptured abdominal aortic aneurysms.
  • To establish a model for future endothelial cell studies in this patient population.

Main Methods:

  • Fat samples were collected from the anterior abdominal wall of five patients with ruptured abdominal aortic aneurysms and five control patients.
  • Endothelial cells lining capillaries were examined using transmission electron microscopy.
  • Photomicrographs were assessed blindly by three independent observers.

Main Results:

  • Endothelial cells in ruptured aneurysm patients exhibited a more convoluted luminal surface with increased projections into the capillary lumen compared to controls.
  • Budding of the endothelial cell luminal surface was observed in 11/45 capillaries in the rupture group versus 0/44 in the control group (P < 0.005).
  • Budding was present in at least one capillary in all five ruptured aneurysm patients but in none of the control group (P < 0.05).

Conclusions:

  • Ultrastructural changes in capillary endothelial cells are evident at an early stage following ruptured abdominal aortic aneurysm.
  • These early endothelial cell alterations may be significant in the pathogenesis of complications associated with ruptured aortic aneurysms.
