Soft tissue coccidioidomycosis on MRI

Summary

Soft tissue coccidioidomycosis can be challenging to diagnose with MRI. Radiologists should consider coccidioidal abscess in differential diagnoses for soft tissue masses, even if infection is not clinically suspected.

Background:

Coccidioidomycosis is a fungal infection endemic to the southwestern United States.
Soft tissue involvement is a less common manifestation of coccidioidomycosis.
Magnetic resonance imaging (MRI) is frequently used to evaluate soft tissue masses.

Observation:

This study presents two cases of soft tissue coccidioidomycosis evaluated with preoperative MRI.
Clinical and initial imaging assessments did not strongly suggest infection in either case.
Retrospective analysis revealed nonspecific MRI findings in one case and findings suggestive of infection in the other.

Findings:

The MRI appearance of soft tissue coccidioidomycosis can be nonspecific.
In some instances, MRI findings may suggest an infectious etiology, such as an abscess.
The study highlights the potential for misinterpretation of imaging findings in coccidioidal infections.

Implications:

Radiologists should include coccidioidal abscess in their differential diagnosis for soft tissue masses on MRI.
Awareness of coccidioidomycosis as a potential cause of soft tissue masses is crucial, especially in endemic areas.
Early recognition of coccidioidal infections can lead to timely and appropriate treatment, improving patient outcomes.
