Drug-associated primary acute pancreatitis

J B Bourke, M B McIllmurray, G M Mead

    Lancet (London, England)
    |April 1, 1978

    Summary

Diuretic use, particularly cyclopenthiazide with potassium chloride, was significantly higher in patients experiencing their first acute pancreatitis attack compared to controls. Further research is needed to confirm if these drugs cause acute pancreatitis.

    Area of Science:

    • Gastroenterology
    • Pharmacology
    • Clinical Medicine

    Background:

    • Acute pancreatitis is a serious medical condition with various potential causes.
    • Understanding drug-induced pancreatitis is crucial for patient safety and effective treatment.

    Purpose of the Study:

    • To investigate potential associations between drug use and the occurrence of acute pancreatitis.
    • To identify specific medications that may increase the risk of acute pancreatitis.

    Main Methods:

    • A case-control study involving 100 patients with their first acute pancreatitis attack.
    • Patients were matched with control subjects based on sex, age, and reason for emergency hospital admission.
    • Detailed drug histories were collected and compared between the two groups.

    Main Results:

    • A statistically significant excess of diuretic use was observed in pancreatitis patients compared to controls.
    • Cyclopenthiazide with potassium chloride and frusemide were the most frequently used diuretics among patients.
    • Associated, though not statistically significant, excesses were noted for digoxin, antihypertensive, and anti-anginal agents.

    Conclusions:

    • Diuretic use, especially cyclopenthiazide with potassium chloride, may be linked to acute pancreatitis.
    • Further clinical and experimental studies are necessary to establish a causal relationship between diuretics, potassium chloride, and acute pancreatitis.
